Since 1992, Community Health Network of Washington (CHNW) clinics have been providing quality health care to Washington state families. Our network of 21 Community Health Centers (CHCs) operates over 190 clinics dedicated to serving our communities and offering tailored services that you won’t find just anywhere such as:

- Multiple care specialties under one roof
- Connections to social supports
The CHNW network also leads innovative efforts to help more Washingtonians connect to health care. Link to Care WA, for example, is a program that aims to advance digital inclusion. Starting in early 2022, Link to Care WA provides a hotline, website, and in-person support to help people access and manage their care online and by phone.
The initial pilot is limited to clients of Yakima Neighborhood Health (YNHS), and additional offerings will be announced as they become available.
